Throughout most of his adult life, Kim Il-sung suffered from a massive grapefruit-sized goiter located just behind his right ear. Murals and paintings featuring the bombastic North Korean leader were always angled to conceal the goiter, and very few photographs showing it exist in order to perpetuate the myth of perfection. This is one of the only known pictures that confirms the existence of the massive lump.
